You may submit your application package for Public/Private Venture Housing (PPV) or Military Family Housing (MFH) prior to departure from your present duty station, however your application will not be effective before you detach. Utilizing your detach date as your start date on the waiting list is valid up to 30 days after your “Report No Later Than” date.
It is important to come into the local housing office once you arrive to assure that your application has been activated. Application will be made on DD Form 1746, Application for Housing.
Applications can be obtained online, from your nearest Navy housing office or call us to have one mailed.

A signed copy of Addendum A - Sexual Offender Notification. |
|
A copy of Permanent Change of Station (PCS) orders or Homeport Change Certificate that brings (brought) you to this area. |

A copy of current Record of Emergency Data (Page II) or DEERS enrollment form listing all family members who will be residing with your family.
|

Once detached, proof of your detaching date. Acceptable detachment paperwork includes: a detaching endorsement, an activity loss statement or an activity history page.
Note: If the military member will be unavailable at the time of lease signing for PPV Housing, a special Power of Attorney will be required for the spouse. Sample PPV Power of Attorney Form
