July 29, 2009 – This Seafair be our special guest and ride aboard a United States Navy vessel. Walk through passageways, climb ladders, tour the different decks, and get a real taste of life at sea. Experience the thrill of riding a Navy ship as it cruises Puget Sound. All cruises depart and return to Pier 30 in Seattle and include lunch onboard the ship. Previous Seafair guest cruises have included destroyers, frigates, cutters and hospital ships; due to heightened security requirements, the ships for this year cannot be identified at his time. Cruises depart from Seattle and may last from six to eight hours, including boarding and disembarking. This is a day you don't want to miss, so fax or mail in your application today!

| Policies and Procedures |
**Please read prior to completing application
This event is open to US Citizens, 8 years of age or older. Children under the age of 8 are not eligible to participate.
1. All applications received by the deadline will be processed at random. Applications must be received by July 10, 2009. All applications must be accompanied with full payment. The Navy and its agents are not responsible for lost or misdirected mail.
2. This application is for up to 7 people only. Complete full name with middle initial and date of birth are required for all applicants. No substitution of riders will be permitted.
3. You will receive your ship assignment, credentials and cruise information packet approximately one week prior to the cruise date. This will include information on parking, clothing recommendations and other pertinent information.
4. Seafair Guest Cruises are open US citizines ONLY who are 8 years of age and older. Minors (ages 8 through 17) must be accompanied by an adult.
5. PLEASE NOTE: These ships are working military vessels and are not designed as passenger ships or configured for persons with ambulatory disabilities. Seating is often limited. Military vessels are exempt from the provisions of the Americans with Disabilities Act. The following physical requirements must be enforced for your safety. Physical requirements include: ability to walk or stand on steel decks for 4 to 8 hours, climb steep and narrow steel ladders, and non-dependence on such items as canes, walkers or casts. Any person failing to meet the physical requirements for riding the ships, at the time of boarding, will be refused access to the ship; no refunds will be given under this circumstance. Final approval authority for all ship riders remains with the ship's Commanding Officer.
6. The cruises last between six to eight hours, depending on vessel assignment, embarkation time and disembarkation time.
7. A $10 fee will be charged for cancellations PER RIDER, prior to July 10, 2009. No refunds will be made after that date. This includes cancellations due to inclement weather. Purchaser of ticket must be in possession of ticket when boarding the ship and/or bus.
8. Last minute changes may occur and we will give our best efforts to communicate these changes to participants. The Cruise Coordinator determines all ship assignments and all assignments are final. Final approval authority for all ship riders remains with the ship's commanding officer.

| Group Information |
Please designate one person as the main contact or group leader for groups of seven or more. This person will receive all communications, ship assignments, credentials and other pertinent cruise information. Group leaders are responsible for communicating all of the above information, including these policies and procedures, to their group. Group leaders will complete ONE APPLICATION and attach a typed list of names (Complete first, middle initial, last) PLUS date of birth of all the people comprising the group (please include group name and leader's name at the top of list). Applications cannot be accepted if this information is not complete.

| Substitution of Riders |
NO SUBSTITUTION OF RIDERS WILL BE PERMITTED.
